Survey from Utah State Health Dept. Lots of health related questions, but no sensitive/identification information. Mostly yes/No questions.Does take 10+ minutes to get through all the questions

This number has called repeatedly over the last several weeks. 8 times in the last week, and three times this evening. They keep saying I agreed to take the survey and when I tell them I did not, they get hostile. We have asked repeatedly to stop calling us. Each time we have told them to stop calling. What does it take to get this stopped?

Just junk call from Utah Dept of Health

A lady from the health department keeps calling my friend at 9:00 at night. Obviously a scam.

I have been called by this number a few times and each time it is after 9:00 pm. I havent answered but have tried to call back immediately after they hang up and it tells me that it is a disconnected phone nu,ber at the Cannon Health Building. This is incredibly annoying and needs to stop.

This number has called me 4 times in two days and even during my working hours. If you call back it says its not a working number. Not sure why I keep getting this call but its VERY annoying to say the least.

This number and the survey seem to be completely legitimate. Somebody needs to shut these idiots down ASAP. They keep calling at all times, especially late at night, and just hang up

Keeps calling during evening between 5 and 7:30pm. Hangs up. Does not leave a message.

I got a fraudulent call from some woman who said she was calling from the Health Department with a survey. She asked if the number she called was a cell phone. She asked if we had a land line too. Other questions about our phone lines were also asked. I called the number back and it turned out to be an inactive number from the health department. Obviously, the scammer was using a spoofed caller ID.

The Utah Health Dept would not make these kinds of calls.The address is 288 North 1460 West and is to Utah office of Vital Reords.Somebody trying to contact people to get info and then request Birth or Death Certificates with info and then Identity theft or false immigrant ID.Call Salt Lake City PD and help me put this dog down>Thanks

This phone # calls , but when I return the call they said this number is no longer in service , what ? it just freakin called ????SHUT THEM DOWN
